COVID Testing for Students Who Become Sick at School
Students need consent from parent/guardian for testing. For families who would like to fill out the consent form, there are two options:
Option 1: Sign Online (recommended at CPS website)
Enter student ID# and date of birth of the child
Select the form from the drop down menu
Option 2: Print & Deliver
If a family doesn't have an Internet connection, the forms can be downloaded for them here. A link will also be added to the system on the Back-to-School portal.
